How does Dynase Nasal Spray work?

Dynase Nasal Spray is a combination of Fluticasone Propionate and Azelastine, used to relieve sneezing and runny nose caused by allergic rhinitis. Azelastine is an antiallergic that eases allergy symptoms like runny nose, watery eyes, and sneezing.

Indication of Dynase Nasal Spray

Pharmacology

Azelastine hydrochloride has an H1-receptor antagonist action, and its main metabolite, desmethylazelastine, also acts as an H1-receptor antagonist. Fluticasone Propionate is a synthetic trifluorinated corticosteroid with anti-inflammatory properties. The exact mechanism of how Fluticasone Propionate works on allergic rhinitis symptoms is not fully understood. However, it is known to impact multiple cell types (such as mast cells, eosinophils, neutrophils, macrophages, and lymphocytes) and mediators (including histamine, eicosanoids, leukotrienes, and cytokines) involved in the inflammatory process.

Dosage & Administration of Dynase Nasal Spray

Adult Dose:

• For seasonal allergic rhinitis, the recommended dosage is 1 spray in each nostril twice daily.

Child Dose:

• For children younger than 6 years old, safety and efficacy have not been established.
• For children 6 years and older, the recommended dosage is 1 spray in each nostril twice daily for seasonal allergic rhinitis.

Administration:

• Shake the bottle gently before each use.
• Prime the bottle before the first use by releasing 6 sprays or more until a fine mist appears.
• If the nasal spray is unused for more than 14 days, reprime the bottle by spraying once or more until a fine mist appears.

Interaction

Drug-Drug Interactions:

• Dynase Nasal Spray may interact with antidepressants (duloxetine, escitalopram), anticonvulsants (pregabalin), HIV/AIDS drugs (ritonavir), antifungals (ketoconazole, fluconazole, itraconazole), anti-allergy drugs (cetirizine), cough relievers (codeine, hydrocodone), and muscle relaxants (carisoprodol, cyclobenzaprine).

Drug-Food Interactions:

• Dynase Nasal Spray may interact with grapefruit juice.

Drug-Disease Interactions:

• Inform your doctor before using Dynase Nasal Spray if you have chickenpox, measles, tuberculosis, recent surgery or injury in the nose, glaucoma, cataracts, herpes infection of eyes, or ulcers or sores inside the nose.

Contraindications

If you are allergic to fluticasone, azelastine, or any other ingredient of Dynase Nasal Spray.

Pregnancy & Lactation

Dynase Nasal Spray, which contains Azelastine Hydrochloride and Fluticasone Propionate, falls under the Category C pregnancy classification. There are no adequate and well-controlled clinical trials in pregnant women for these specific drugs. If considering its use during pregnancy, it should only be done if the potential benefit to the mother outweighs the potential risk to the fetus.

Regarding breastfeeding, there is limited information on whether Dynase Nasal Spray is excreted in human breast milk. Since many medications can be secreted in breast milk, caution should be exercised when administering this medication to nursing mothers.
